Doug was born and raised in Mesa. He graduated from Mountain View High School in 1993, then spent one year studying at the University of Utah before leaving to serve a church mission in Finland. After returning, he studied philosophy at Brigham Young University in preparation for medical school. During that time he met and married Tiffany. He graduated from BYU in 2002 and they moved to New Orleans to attend medical school at Tulane University. They had their first child there and also experienced Hurricane Katrina. They evacuated the hurricane and went to Salt Lake City where they had their second child and finished medical school. After graduation, they moved to Portland, Oregon, where Doug did his Internal Medicine residency training at Oregon Health and Science University (OHSU). He was in the Primary Care Track, obtaining special training in the team-based care model.

Upon completion of his medical residency, Doug and Tiffany moved their family to Roosevelt, Utah, taking a position with Uintah Basin Medical Center. He worked there for 3 years, gaining valuable experience caring for patients in both the clinic and hospital. They then moved to Logan, Utah, and Doug started work at Intermountain Healthcare’s Budge Clinic—a multi-specialty group practice. It was there that their third child was born. He worked there almost five years before deciding to start his own practice in Arizona.

Doug has been interested in medical system design since the beginning of his training. He has experienced the many deficiencies of our current system and has long wanted to redesign the care process. Throughout his career, he has studied the system design literature, consulted with numerous experts in the field, and obtained formal training in Lean management systems and primary care design. This practice is the culmination of that preparation, and he is passionate about making a real difference in the lives of his patients and in the medical community.

Hilary Hellman is President of Ancillary Care Strategies. Her passion is to partner with client organizations to facilitate their transformation in even the most challenging of economic and healthcare environments. With 12 years as a Founder/Principal of Ancillary Care Solutions and leveraging more than 25 years of healthcare management experience, Hellman has been a principal at several start-up companies, including a long-term care facility, a leading-edge pharmacy and disease management company, and a healthcare software enterprise. Hellman earned a Master of Arts Degree and Bachelor of Arts Degree in Speech-Language Pathology and Audiology from San Diego State University.

Laurie Fiorica is a dynamic and engaging seasoned executive as well as an expert in operational excellence. She has a 30+ year track record of substantial and sustainable business results. As Vice President of Ancillary Care Strategies, she is a pioneer in Lean in Healthcare and has led transformations in multiple industries, most recently as CEO/COO of OrthoArizona (OAZ). Laurie is passionate about helping teams and organizations dramatically improve their processes, their value proposition, develop their cultures, and achieve outstanding results.

Audra is an innovative healthcare Leader and Team Builder, Driving Operational Efficiency and Strong KPI Improvements. She has served as a VP/Chief Accounting Officer, Controller, and CFO for domestic and international businesses spanning diverse industries. Audra has provided Financial Statement, Operational, FP&A, and M&A leadership for both private and publicly held companies, managing teams from 10 to up to 90 including holding financial oversight of a $1.5B company. She excels at building empowered, collaborative teams focused on operational excellence. Developing forward-thinking strategies and innovative solutions is her specialty. She has built teams that have delivered breakthrough gains in revenues, profits, and other KPIs throughout her career. Audra is dedicated to maximizing shareholder value and achieving operational and financial objectives.

Abby Abby is an occupational therapist and board-certified health coach with a passion for whole-person, mind-body medicine.

Throughout her experiences working in an inpatient psychiatric hospital, home health, private practice and within a fully remote diabetes and weight-loss clinic, she has supported individuals with a variety of complex physical and mental health needs.

Abby’s interest in whole-person care began while observing many of her loved ones have difficulty managing chronic illnesses, feeling overwhelmed by their healing process, and finding themselves “stuck” in habits that did not align with their health goals. This led her to help others find individual pathways to wellness.

As a former collegiate athlete, Abby grew intrigued with the mind-body connection while witnessing many of her teammates recover and manage injuries and pain with comprehensive training around mindset, movement, sleep, stress, and rest/relaxation. Abby enjoys helping individuals adopt healthy habits and routines around these concepts through listening and open questioning while engaging in health promoting activities to learn through doing.

Abby played volleyball and received both undergraduate and graduate degrees at the University of New Hampshire. Abby grew up in Ohio and has since lived and worked in New Hampshire, Washington, Colorado, and Arizona. Outside of work, Abby spends her time at yoga, hiking, running, swimming or in nature. She treasures quality time with her cat Millie, her friends and family.

Jasmyn grew up in Las Vegas and attended UNLV where she earned numerous academic awards and a degree in Nutritional Studies. She worked for a few years as a nursing assistant, then attended A.T. Still University in Mesa and received her Physician Assistant degree in 2020. She worked with a COVID mobile unit before joining Maready Medical. She enjoys exercising, baking, water sports, hiking, among many other things.

Jasmyn is passionate about her medical work and is meticulous in providing her care. Her comfortable interaction style makes it easy to work with her. Her range of medical knowledge is excellent. With her background in nutritional science, she is able to incorporate good nutrition and physical activity into healing disease. She understands human nature and can relate to people and their problems while still motivating people to become healthier. She is accepting patients for general care and can also provide well-woman exams.

Lexi is a registered dietitian nutritionist (RDN) who is focused on helping people accomplish their nutrition goals. She is passionate about supporting people in achieving optimal health by using evidence-based medical nutrition therapies to prevent and treat disease. Her mission is to inspire people to form a healthy relationship with food to improve their daily life and last them a lifetime.

Lexi received a Bachelor of Science in Nutrition and Food Sciences from Louisiana State University and a Master of Science in Exercise Physiology (Sports Nutrition) from Florida State University. She was born and raised in New Orleans, Louisiana where she was a competitive cheerleader for 16 years. Her hobbies include traveling, group fitness classes, hiking, cooking, and what she likes to call “walk ‘n talk” where she goes on walks and talks to her family and friends back in Louisiana.

Dory is PA who graduated from Midwestern University in 2023. She is a California native and attended the University of Southern California and received a degree in Health and Human Sciences with a minor in Spanish. She studied abroad in Guatemala and Spain during that time and was able to further her Spanish speaking abilities and discover her passion for travel and exploration. She worked for 2 years as a urology medical assistant in San Diego, CA before moving to Phoenix for PA school. At Midwestern she was part of the intramural volleyball team, PA liaison for the Medical Spanish Club, and active in student government.

Dory is passionate about personal wellness, nutrition, fitness, and happiness. She is an advocate for finding the root of medical problems to promote overall health and prevent disease. She loves connecting with other humans and values her relationships with her patients. Her hobbies are baking, traveling, exercise, volleyball, and her favorite part about living in Arizona is hiking next to cacti.

Anita is a physical therapist with over 30 years of clinical, education and healthcare management experience. She has provided traditional and specialized patient care. She is recognized for professionalism, patience, positive mental attitude, and commitment to excellence. She enjoys working with physicians, staff, patients, and families to improve the physical well-being of her patients. In addition to treating, she has held multiple senior-level positions overseeing clinical operations and therapy practices across the country.

Anita attended Ithaca College and previously held Board Certification Specialist in Sports physical therapy for over 20 years.

Anita is passionate about the role that movement and physical activity plays in the lives of her patients – of any age or situation. She raises horses on her ranch in N. Scottsdale, AZ and competes in cowboy-mounted shooting recreationally.

Wanda Ichiriu PA-C is a board-certified physician assistant with over 26 years’ experience in internal medicine/primary care, urgent care/emergency department, and nephrology. She has had 21 years’ experience in nephrology to include management of both out-patient and in-patient/hospitalized end-stage renal disease/hemodialysis and chronic kidney disease patients. She earned her Bachelor of Science degree in physical therapy at California state University, Long Beach and earned her postgraduate certificate from the Yale School of Medicine Physician Associate Program. Prior to becoming a physician assistant, she worked as a physical therapist for 7 years prior to enrolling in the Yale PA program. As a physical therapist, Wanda has worked in a variety of settings/subspecialties to include outpatient/inpatient orthopedics musculoskeletal injuries, wound care clinic, burn intensive care unit, cardiac rehabilitation, acute rehabilitation to include stroke rehabilitation and status post amputations, general medical/surgical unit, post cardiothoracic surgery patients to include critical care cardiothoracic surgical unit, and skilled nursing/long term care facility patients.

After working over 20 years as a nephrology physician assistant, Wanda left the specialty to pursue her true passion, which is an integrated, functional medicine approach that focuses on trying to find and treat the root cause of the disease . This incorporates a collaborative approach with provider and patient and treating the patient as a whole person , addressing physical, mental/emotional and spiritual influences. Currently, Wanda is working on the Institute for Functional Medicine Certified Practitioner (IFMCP) certification with anticipated completion in 2024. She is genuinely caring and personable, and serves her patients with unparalleled attention to detail, dedication, and compassion.

Wanda is a recent Arizona transplant from California and loves the warm climate and unique desert terrain. She enjoys outdoor activities to include hiking, gardening, and nature walks. Her hobbies include cooking, such as multi-ethnic cuisine, healthy cooking, and baking (Japanese rice cake/mochi desserts is her specialty), and cultivation of exotic and rare fig and dragon fruit trees.

Jodi is a registered yoga teacher, licensed yoga instructor, professional dancer, choreographer, fitness instructor, nutrition and cooking guru, and credentialed as a Dance Master, America.

Jodi was a successful advertising executive and left to pursue her passion for inner balance and healthy lifestyle through yoga, dance, exercise, mindfulness, and healthy eating. She successfully blended these passions to overcome her life-threatening battle with Crohn’s disease. She learned to manage this disease naturally and has maintained medication-free. Jodi has extensive experience with our current medical system and believes so strongly in the mission, that she joined Forte to participate in drastically improving how our medical community supports wellness rather than sickness. Clients are drawn to her for her vitality and love of life, and for her honesty, kindness and true concern for their health, inner peace and well-being.

As a coach and mentor, Jodi guides with a holistic approach to mental and physical well-being through whole food diet, exercise, and stress management tailored to their lifestyle. She supports, motivates, and empowers her students through education, research, and support.

As a mother of two children Jodi lives her life leading by example, with an open heart, love and compassion for all. She is actively involved in her community as a board member of the Boys and Girls Club, and a committee member of Catholic Family Center.

Jodi attended Ithaca College and State University of New York (SUNY) College of Geneseo where she graduated with a degree in Communications. She is a Principle, American Dance and Yoga Academy. Jodi holds the following certifications: RYT, LYI, mindfulness and meditation and Yoga fusion. She is also a Yoga, Mindfulness , and Fitness consultant for the Rochester City School District. Jodi resides with her family in NY.

Martha is a board-certified physician associate with over 22 years of combined experience in family practice, internal medicine, urgent care, and as a hospitalist. She is an Arizona native who completed her undergraduate degree at Arizona State University, then attended law school in San Fransisco before realizing medicine was her true passion! She did some of her medical pre-requisites at the University of the Virgin Islands in St. Thomas, and then completed her postgraduate medical training at Midwestern University where she graduated Cum Laude.

Martha’s approach is to look at the patient as a whole. She takes pride in educating her patients on living a healthier lifestyle thru better nutrition and personal health awareness with a focus on being up-to-date with routine health screenings and other important health maintenance items.

Martha stays physically and mentally fit with regular exercise, yoga and meditation and also enjoys hiking, kayaking, cooking and travel. Martha is currently accepting new patients.
