Chantelle Iloreta, PA-C

Chantelle joined Forte in early 2023. She was heavily influenced to pursue a higher education in healthcare to pay tribute to the perseverance and sacrifice of her grandparents that enabled her family to immigrate from the Philippines to the United States. She is dedicated to primary care for her patients, including women’s and men’s health. She is an active listener and puts patients at the center of her practice. Prior to joining Forte, she provided comprehensive preventative testing at an integrative medicine practice. Beginning her clinical experience as a medical assistant and medical scribe, she progressed to a leadership position in an underserved community. Chantelle graduated from West Coast University, completed her B.S. Biology from Grand Canyon University (GCU), and was born and raised in Waipahu, Hawaii. Chantelle was a former student-athlete who enjoyed spending her summers in Hawaii volunteering as an assistant coach for her former competitive volleyball club, Mililani Volleyball Club. During her free time, she enjoys cooking, hiking, taking long walks with her dog, Oliver, and spending time with her family.
